COURSE LEARNING OBJECTIVES

Upon completion, successful students will be able to:

  • Understand the philosophy, process and procedures required to support a marine casualty investigation.
  • Exercise the techniques of collecting, analyzing and reporting on marine casualties.
  • Ensure that the root causes and factors contributing to an incident are identified using various techniques including the 5Whys.
  • Build on Resilience vs. Brittleness on the outcome of the investigation report.
  • Outline a project plan to implement TMSA / Dry-Bulk Requirements for Incident Investigation.

Delegate Assessment – Assessment is carried out by continuous assessment of delegate performance throughout the theoretical and practical phases.

 

AIM OF THE COURSE

This course aims to provide participants with tools, methodologies and concerns to ensure effective reporting, investigation and analysis of incidents, and near misses.

Successful completion of this course will enable participants to ensure that where an incident or accident occurs, the subsequent investigation is thorough, the root causes are identified and that measures are implemented to prevent a recurrence and communicated effectively to shore-based and vessel personnel.

 

Prerequisites

Participants are recommended to have the following prior knowledge before attending this course:

  1. Casualty investigation Code (CI Code) (resolution MSC.255(84);
  2. IMO Assembly Resolution A.1075 (28) – Guidelines to assist investigators in the implementation of the Casualty Investigation Code (CI Code) (Resolution MSC.255 (84));
  3. MSC-MEPC.3/Circ.4 – Revised harmonized reporting procedures – Reports required under SOLAS regulations I/21 and XI-1/6, and MARPOL, articles 8 and 12;
  4. IMO Assembly resolution A.1070 (28) – IMO Instruments Implementation Code (III Code).
  5. Guidelines for the application of the Human Element analysis process (HEAP) to the
  6. IMO rule-making process, MSC-MEPC.2/Circ.13
